The Ministry of Health, Labour and Welfare is making efforts to promote patient-centered community healthcare by clarifying the division of roles according to the functions of medical institutions and by promoting cooperation between medical institutions. In keeping with these efforts, our hospital is further promoting mutual division of roles and cooperation through discussions with local medical associations and other organizations. For this reason, we ask all first-time patients to bring a letter of referral from their regular physician.
First-time patients are asked to bring a referral letter from a hospital, clinic, or other medical institution if at all possible. Please note that those who do not have a letter of referral will be charged a separate fee for medical care not covered by insurance (7,700 yen) for their first visit.
This is a fee for expenses not covered by medical insurance charged separately from the first-time visit fees when a patient receives medical care for the first time at a regional medical care support hospital with 200 or more beds without a referral letter (medical information form). (Medical and dental services are handled separately.) Additionally, you will be required to pay if you had received medical care previously but had not continued treatment.
Procedures are carried out at counters 4 & 5: First Visit Reception. Fill in the necessary information inside the red frame of the medical examination application form and turn it in to the First-Time Visit Reception Counter, along with your Residence Card or passport, and your Health Insurance Card, etc. if you have one, in order to have your medical records (electronic) and Patient Registration Card (ID card) made.
For patients with a referral letter, procedures are carried out at counter 3: Referral Patient Counter
Please wait in front of the Examination Room until your turn arrives.
After your examination, hand the file containing your payment slip, prescriptions, etc. to counter 6: Payment Counter.
You will be given a number card, and when your fees have been calculated, your number will be displayed.
Take your Patient Registration Card and pay at the automated payment machine. (Hospitalized patients use the barcode on their invoice instead of their Patient Registration Card.)
Your next appointment for testing, examination, etc. will be decided at the end of your examination. You will be issued an appointment voucher upon payment. Please bring it with you on your next visit. On the day of your next appointment, check in at the return visit reception machine, and then go to receive your test, examination, etc. (When you arrive in front of the examination room, please put your Patient Registration Card in the card pocket near the door and wait.)
*Points of Note
To change the date of your appointment, please contact the Appointment Center before the date of the appointment with your doctor.
TEL: 075-533-1295 (direct line)
Emergency care is available outside of reception hours, at night, and on holidays. However, you will be treated by the doctor on call that day. Please note that the order in which patients are received may vary due to the state of the patient or the department where examination is taking place.
Medical certificates are written and issued at a later date, after a medical specialist examines the patient during regular examination hours.
Our hospital is a tertiary emergency medical care center. If a patient in a critical state is brought in, you may have to wait. Please visit your nearest doctor’s office during its opening hours or a holiday emergency clinic, etc., if at all possible. Please also note that non-urgent examinations are available at the outpatient clinics of each department during weekday hours.
